How reproducible:


Steps to Reproduce:
I have launch few deployments with mixed state with few instances as running and few failed.

For deployments with mixed state,the provider name is not displayed in the provide column.
see attached screenshot.

  
In instance list, if any instance fails, then provider name is not displayed for that failed instance, but in deployments of mixed state we do have running instances.


Expected results:
Provider name should be displayed if deployment state is mixed as it as running instances.
