Description of problem:
* When the user ran `rhc setup`
* After entering the user account password, the CLI printed the error message "stty: standard input: unable to perform all requested operations"

(see attached screen shot)

Version-Release number of selected component (if applicable):
* rhc version unknown
* ruby version (from conversation on #openshift): ruby 1.9.3p286 (2012-10-12 revision 37165) [x86_64-darwin12.2.0]
* OS version: OS X 10.8 (Mountain Lion)

How reproducible:
Could not be reproduced with rhc version 0.98.16 or 1.0.4 with same ruby build and OS.

Seeing this on old commander versions - all our Commander 4.0.3 builds on RHEL are displaying this (1.9.3 especially).  Recommend user should upgrade beyond 4.0.3 (only fixing major functional bugs in 4.0.3 at this point) and that we should KB this.
