Description of problem:
this issue was separated from https://bugzilla.redhat.com/show_bug.cgi?id=877836#c28 
Fail to initialize the the data disk specified x-data-plane=on via 'Device Manager' in win7 64bit guest using the RPMs v3, with the prompt error 'The request could not be performed because of an I/O device error', i will atttach the log of screenshot later.

BTW, if specified x-data-plane=off to the data disk, it can be initialized and formatted successfully.

Expected results:
it can initialize and format the data disk successfully.

Verify this issue qemu-kvm-0.12.1.2-2.352.el6 with the same steps as comment #0.

host info:
kernel-2.6.32-355.el6.x86_64
qemu-kvm-0.12.1.2-2.352.el6
guest info:
win7 64bit
virtio-win-prewhql-0.1-49 

Results:
it can initialize and format the data disk successfully without any problem.

Base on above, this issue has been fixed successfully.
