Description of problem:
I'm seeing this problem repeatedly, and can now say with reasonable certainty that it seems to happen only immediately after resuming the system from a suspended state.

Contrary to my original comment about this problem seeming to come always after a suspend-resume, I've just seen it happen after a power-up from a powered-off state.

This issue always seems to coincide with bug 905091.
