Description of problem:
When we try to add new JVM configuration from management console in domain mode "Add" button is not working looks like its disabled.

Version-Release number of selected component (if applicable):
EAP 6.0.1 (Affected Version)

How reproducible:

 
Steps to Reproduce:

1. Start JBoss EAP 6.0.1 in domain mode using domain.sh script.
2. Login to admin console
3. Go to "http://localhost:9990/console/App.html#host-jvms"
4. Click on "Add" button,but it don't show any window to add new JVM configuration.

In JBoss EAP 6.0.0, we can add new JVM configuration from admin console.

I cannot reproduce this bug on 1.5.1 / AS 7.2

Verified - add button is now behaving correctly in 6.1.0.ER3
