Ever since RHEL 6.0 you're not supposed to directly use 'host_device' as a
format any more. Simply using 'raw' does the expected thing. The man page still
mentions that it should be used for conversion targets. This should be removed.

qemu-kvm-0.12.1.2-2.356.el6.x86_64:

man qemu-img :

       host_device
           Host device format. This format should be used instead of raw when converting to block devices or other devices where
           "holes" are not supported.


qemu-kvm-0.12.1.2-2.376.el6.x86_64:

man qemu-img :

"host_device" part has gone.
