Description of problem:
GSS Requirements section of hardware certificates has following statement:

A representative sample of this hardware has been sent to Red Hat under the terms of the Hardware Requirements agreement. 

Followed by "Current Status: No Yes" check boxes. 

Here we are basically checking whether a sample hardware has been sent or not, shouldn't the above statement be phrased in a question format e.g.:

Has a representative sample of this hardware been sent to Red Hat under the terms of the Hardware Requirements agreement?

Also, when "No" is selected in the "Current Status:" section, it does not let us to modify  "Shipping Information". Right now while providing a TAM ack to a certificate where a sample hardware has not been sent, we select "Yes" check box and then enter following line in "Shipping Information":

Red Hat may ask for sample hardware in future for testing.

Certificate should allow us to post above line in "Shipping Information" box when "No" is marked in "Current Status:" section.

Open any certificate e.g.:

https://hardware.redhat.com/show.cgi?id=1003718

And check "Gss Requirement" sections in it. 

Actual results:
A representative sample of this hardware has been sent to Red Hat under the terms of the Hardware Requirements agreement. 

"Shipping Information" box is grayed out when "No" is selected in "Current Status:" section. 

Expected results:
Has a representative sample of this hardware been sent to Red Hat under the terms of the Hardware Requirements agreement?


"Shipping Information" box should not be greyed out when "No" is selected in "Current Status:" section.

the supposed action should be when the yes selected, then you can input the shipping information.
