packstack --allinone terminates with:

                                                                                                [ ERROR ]

ERROR : Error during puppet run : Error: Could not prefetch glance_image provider 'glance': Execution of '/usr/bin/glance -T services -I glance -K 72a37cd098824aa5 -N http://192.168.0.181:35357/v2.0/ index' returned 1: Error communicating with http://192.168.0.181:9292 [Errno 111] Connection refused
Please check log file /var/tmp/packstack/20131009-135102-ooulfW/openstack-setup.log for more information

Instructions at http://openstack.redhat.com/Workarounds#Could_not_prefetch_glance_image_provider_.27glance.27 suggest re-running, but this results in exactly the same error condition.

Rich, I'm not seeing this on the many runs we've done.  It seems that keystone didn't start in this instance, but it's not clear from the log extract why that was.  

If you can reproduce it and we can get the keystone logs, please reopen and we can investigate.
