Description of problem:
Currently the documnetations describes the use of the RHEV-M Appliance self contained on RHEV-M, RHELOSP and VMWare.

This is incorrect and the documentation should describe and recommend only how to use the RHEV-M Appliance in the context of hosted-engine-setup.

1. Remove the recommendation to use the RHEV-M Appliance with RHEV and RHELOSP
2. Document the use of the RHEV-M Appliance in the context of self-hosted-engine

During hosted-engine setup you will be asked if you want to use an existing disk, that is
the time where you need to point the HE setup to the appliance.
