firefox-42.0-2.fc23.x86_64 In the Library, I enter a bookmark folder which happens to be a subfolder of a folder under Bookmark Toolbar. After clicking around on existing bookmarks, I select Organize -> New Bookmark. After filling in the Name and Location fields, I click away to do other things. I notice that the new bookmark I created has the logo from one of the existing bookmarks, despite being in a different domain. This also happens when I select "New Bookmark" from the right-click menu.
The incorrect icon will sometimes go away if the bookmarked page is visited, though not all the time. I think this depends on whether or not the page supplies a favicon.
