Description of problem:
[UI] - External Providers -> Edit Provider - 'Interface Mappings' field is not aligned in the Agent Configuration sub tab.

The 'Interface Mappings' field is not aligned as it should and it's look a bit broken in the edit provider dialog. 

Attaching screenshots to compare between 4.0 and 3.6

1. Open Edit Provider dialog under External Provider tab and choose the Agent Configuration sub tab

Moving from 4.0 alpha to 4.0 beta since 4.0 alpha has been already released and bug is not ON_QA.

Hi Dominik,

I can confirm that this bug is no longer exist on latest master 4.1 - 4.1.0-0.0.master.20161112231308.git672bd31.el7.centos
