Description of problem:
I was doing a RHV(self hosted, 4 host) + OSE(1 controller, 2 workers) + CFME deployment.   The deployment failed while installing RHEV on the remaining hosts (i.e. the first host for the self hosted VM succeeded).   3 of the hosts succeeded in installing RHV but one of them did not finish and thus the deployment timed out:

ERF42-7017 [Foreman::Exception]: You've reached the timeout set for this action. If the action is still ongoing, you can click on the "Resume Deployment" button to continue.

When I looked at the screen of the host it had this on its screen:

anaconda 21.48.22.56-1 for Red Hat Enterprise Linux 7.2 started.
 * installation log files are stored in /tmp during installation.
 * shell is available on TTY2
 * when reporting a bug add logs from /tmp as separate text/plain attachements

In the syslog it had the following error and warning:

How reproducible:
intermittent, only seen once.

Steps to Reproduce:
1.  Do a self hosted RHV deployment with 4 hosts

Actual results:
In this one case anaconda just sort of stopped with no indication of 
why in the logs.

Expected results:
This not to happen.

Note this bug is more of a place holder for people to add notes if they see this again.  The event happened.  It wasn't expected or wanted, and this records this event.
