Description of problem:
=============================
SSIA, the host has a pending approval state.  CF picks that up but doesn;t have a icon for it.  


Version-Release number of selected component (if applicable):
Verified present in 5.5.5 and 5.6.1


How reproducible:  100%


Steps to Reproduce:
1.  Manage rhev provider with host in 'pending approval' state
2.  view providers host quadicon list 
3.  view host details

Actual results:  missing icon


Expected results:  pretty icon
