Description of problem:
Can not bring host2 to up to running state. Failed

Steps to Reproduce:
1. I updated os and vdsm on host2.
2. Trying to activate host2.
3.

Actual results:
Activating of host vochomurka failed with error: 
Host vochomurka cannot access the Storage Domain(s) <UNKNOWN> attached to the Data Center DCHK. Setting Host state to Non-Operational.

I see in error vdsmd log on host this error:
MiscBlockReadException: Internal block device read failure: 'name=/dev/69487b75-090d-40c0-8a12-50f90b7aad75/metadata, offset=0, size=2048'

But i i try read it, it is readable
dd if=/dev/69487b75-090d-40c0-8a12-50f90b7aad75/metadata of=/dev/null bs=1M
512+0 records in
512+0 records out
536870912 bytes (537 MB) copied, 4.21475 s, 127 MB/s


Expected results:
second node(host) up and running to serve vm's

Additional info:
I think all iscsi resources is available

I reinstaled affected host to Ovirt node 4.0.3 and it started working.
