If you log into the new bugzilla 5.0, with a wrong password, you will be directed to a page telling you to 'Press back and try again'

In the top right of the page you will have a login button.  When you click on this it appears to be broken.  It shows you a 'Create account' button, but nowhere to enter a username and password.

In the old interface this screen wouldn't present you with an option to login, which is probably the source of the breakage.

Tested on QA environment(5.0.3-rh8)
Result: Pass
