USB controller fails to initialize correctly. xhci_hcd can't setup: -110

xhci_hcd fails to initialize. USB ports disabled.
Version: Linux version 4.11.11-300.fc26.x86_64 (email@example.com) (gcc version 7.1.1 20170622 (Red Hat 7.1.1-3) (GCC) ) #1 SMP Mon Jul 17 16:32:11 UTC 2017
How reproducible: Always
Steps to Reproduce:
1. Completely power down PC
2. Plug Mouse and Keyboard into an Intel Series 9 chipset USB port (either USB2 or USB3).
3. Power up PC. Mouse and Keyboard work in BIOS.
4. Grub screen appears. Mouse and Keyboard continue to work.
5. System begins to boot. Mouse and Keyboard turn off.
6. See error msgs in journal (attached):
xhci_hcd 0000:00:14.0: can't setup: -110
xhci_hcd 0000:00:14.0: USB bus 3 deregistered
xhci_hcd 0000:00:14.0: init 0000:00:14.0 fail, -110
xhci_hcd: probe of 0000:00:14.0 failed with error -110
Actual results: Mouse and Keyboard disabled. All USB ports on main USB controller inactive. Mouse and Keyboard work on secondary USB controller (ASMedia-based).
Expected results: USB controller to initialize and work.
Motherboard: ASRock Z97 Extreme 6
Have tried various combinations of BIOS USB options. Currently using USB defaults.
BIOS flashed to latest version.
