A customer has reported via the cloudforms-docs mailing list that a known issue in the 4.5 HA Guide is no longer an issue. Remove this note.

The note pointing to a bug in this installer is no longer relevant in 4.5, the installer does indeed prompt to format any database volumes.  Let me know if ?s.
