I'll take a look at the vmcore.  

As far as the live system failure:

(1) The TESTOUT.log doesn't show anything of use besides the grep of the
    error message.  What am I supposed to do with that?
(2) But more importantly, a bt error that can happen any time on a live system,
    because the stack contents of a process may be changing while the 
    command is being run.
