Description of problem:
The 4.0 Release notes incorrectly state under the ceph-ansible section that before starting the upgrade process you must update osd_scenerio to lvm and to remove any devices previously used by ceph-disk from the list of devices. 

This is not needed and we actually do not support the osd_scenerio option in RHCS 4.0.

Ceph-volume simple scan is actually used and the existing OSD's deployed by ceph-disk are taken over by ceph-volume, this is the same for containerized services as well as the deployment is done in the container-image itself.
