The root cause is someone cloned a partition table (using dd?) and now there are multiple disks whose partition tables have the same UUID. Obviously, UUIDs are supposed to be unique. We can explore options for a workaround on our end, but a good practice on the customer side would be to not duplicate UUIDs.

Closing this issue, partly because of a lack of information/updates. Duplicate partition table UUIDs should be addressed in the tool that clones disks based on the template.
