I saw this issue in ODF4.12 now. Not tried with 4.11 but having plan to check with ODG 4.13.

I have not seen this issue on ODF 4.13.
Mon pods are running after following procedure.

Rook's internal code doesn't call the method to delete auth for OSDs. Something else must be happening to result in those auths being deleted. Possibly, the mons lost some info or got corrupted during the mon cluster failure?

We are still waiting to hear back about Parth's question, "Is cephclient enabled in the cluster?" That is the only place where Rook might (unlikely) have deleted those auths.

@asagare do you have updates? If not, I don't know what more there is to be done, and the PR will be closed in a week or two.
