We need to move JBoss legacy entitlement administration from SEP to Issue-Tracker to enable JBoss technicians to manage this data set without the need for an additional login/application.

Presentation format can follow existing admin-type functions.

Three main screens are required:
1) List screen
2) Edit screen
3) Create screen (virtually same as edit screen)

LIST screen:
columns: SKU, Name, Mapped Product, Level
actions: edit, disable

EDIT/CREATE screen:
Fields:
1. Id (read-only, will be blank in create state)
2. Product SKU (text, 30)
3. Entitlement Name (text, 30)
4. Product (dropdown, list of all products)
5. Service Level (dropdown)
- values: Basic, Standard, Premium, Enterprise, Professional
6. Initial Sev 1: (dropdown)
- values: 1 hour, 2 hour, 4 hour, 1 business hour,  2 business hours,  4 business hours, 1 business day, 2 business days
7. Initial Sev 2: (dropdown)
- values: 1 hour, 2 hour, 4 hour, 1 business hour,  2 business hours,  4 business hours, 1 business day, 2 business days
8. Initial Sev 3: (dropdown)
- values: 1 hour, 2 hour, 4 hour, 1 business hour,  2 business hours,  4 business hours, 1 business day, 2 business days
9. Initial Sev 4: (dropdown)
- values: 1 hour, 2 hour, 4 hour, 1 business hour,  2 business hours,  4 business hours, 1 business day, 2 business days
