Description of problem:
$Summary, because only Intel can create their own we can add this checkbox in the not checked state. We should also print the submit button but then onload use JS to disable it if the checkbox isn't checked. The code should error if the box isn't checked and submit is checked (eg. if js is disabled and the cgi executes anyway). Basic checkbox input should be fine label could be something similar to "I have read the policy guide and confirm this model is a subset and follows the policy." something like how it's done on the create page.
