I just attempted the creation of a minimal livecd.
The packages for the image are downloaded and installed correctly on the
ext3 image; however the script does not manage to umount
the partition and crashes. As the script terminates,
the temporary directory is not cleared and the loop partition
appears to be still mounted; however it seems there are no other
processes accessing it (and a simple umount now succeeds).


Expected result: creation of a livecd image of fedora
Actual result: 800Mb of temporary files under /var/tmp :(

We have fixed up some of the umount issues, so that in most cases even if they are done out of order things get unmounted in the end. However there is still an issue if you kill livecd-creator (e.g. control c it).
So that part of you bug duplicates others.
As for why initial failure occurred, I didn't see a cause in the traceback. Maybe a filesystem ran out of space or too many loop devices were tied up. But perhaps there was a real bug. Do you have any more information on what caused the initial failure?
