Ah ha. See this note in the biosdevname -d output: Duplicate: True That means it was going to try to assign the same name to two different devices, but instead of doing that, it didn't rename either. Congrats - you managed to trip up a race that I hadn't seen before.
I'm unable to reproduce this with the igb card I have available to me. This may be resolved in biosdevname 0.3.7. Please test with that version which I built today and will be in updates-testing and rawhide tomorrow, or grab it from http://linux.dell.com/biosdevname/permalink/. Thanks, Matt
