Description of problem: pthread_mutex_init() and pthread_cond_init(). vdsm should not carry on happily in that case. Also, pthread_{mutex,cond}_destroy() must be called when the mutex/cond falls out of use. Tested with 16 Vms. Each Vm has 3 HDs. Verified ic122 vdsm-4.9-73.el6.x86_64
