Can you retest this bug with virt-who-0.5-3.el5? Bug 769177 looks very similar and is fixed in that version.

I retest the bug on rhel5.8-snapshot2.0(xen).
The result is pass.

The step as following:

1. Install XEN,register to candlepin.
2. Install three guest on xen,and  run the guests 
3. Configure the virt-who.
    # vi /etc/sysconfig/virt-who
    VIRTWHO_BACKGROUND=1 (enable debugging)
    VIRTWHO_DEBUG=1 (run in background)     
4. Restart the virt-who service in terminal1 
    # service virt-who restart
# service virt-who restart 
5.Restart the libvirtd ,and then shoutdown one guest(rhel5-32)
6.Check the output info of terminal1
