Description of problem:

With IE9 one of the fields on the provider form is mis-aligned.  Not a big deal, only cosmetic.

IE8 looked fine as well as FF.

I am unable to reproduce this bug in IE9 or any of the other browsers mentioned in Dave's comment above.  I believe the alignment issue has been fixed in the revisions of the page since the bug was logged.  Please retest when you're able to.
