1.  Go to organizations
2.  select an org
3.  click new environment
4.  fill out name and hit enter

result:  page fully refreshes to a blank page, environment is created.

expected result:  same clicking create.

    799523 - Fix for new environment full page load.
    
    Also greatly simplified the JS and HAML for this page so that the overall experience is better and faster.

getting rid of 6.0.0 version since that doesn't exist
