Description of problem:
DocBook identifies appendixes with capital letters. However, in the built HTML and PDF outputs, the gentext word "Appendix" and the appendix identifier are missing from the title of the appendix. (sections within the appendix receive their correct identifier though), and the HTML <title> tag in the <head> also correctly receives the "Appendix" gentext and identifier

Steps to Reproduce:
1. Build a book with at lease one appendix
2. Look at the built appendix
  
Actual results:
The title displayed is only the title of the appendix, but the appendix is not labelled as an appendix, nor is the particular appendix identified. 

For example, in the Publican User Guide, "Disallowed elements and attributes" -- http://jfearn.fedorapeople.org/en-US/Publican/3.0/html/Users_Guide/appe-Users_Guide-Disallowed_elements_and_attributes.html

Expected results:
The title should be prepended with the word "Appendix" and the identifier for that appendix, for example: "Appendix A. Disallowed elements and attributes"

Hi Rudi, I think this was fixed in 3.1 when we removed all the changes for did for IDs.
