Description of problem:

If you are adding/editing a provider account in the new ui you've already  picked a provider.. 

We can drop the combo box and the choose button

combo box and choose button is removed.

removing bugs from ce-sprint from the tracker.. you can find these bugs by querying the "qa whiteboard" for ce-sprint-60
