Description of problem:
Custom Automation instances cannot be added to an Action in Control Policies. WebUI is missing forms from previous version to allow user input.

How reproducible:
Always

Steps to Reproduce:
1. Navigate to Control -> Explorer -> Actions -> Configuration -> Add a new action 
2. Enter the name of the action 
3. Select Action Type "Invoke a Custom Automation"

Actual results:
A page is displayed showing the title "Object Details" with a surrounding box only.

Expected results:
Should display a form to allow user input to enter name of automation instance similar to 3.1.
