1. Provision a VM on RHEV
2. I tried provisioning on different RHEV providers(atleast 4) 
3. and all requests failed. Logs are attached.

Shveta - We saw this at a customer site a few months back and worked directly with GSS (I do not think a BZ was ever opened on it).  It turned out that their templates were bad after upgrading rhev from 3.2 to 3.5.

They could not provision the templates when they tried directly from the rhevm UI.

Please try to provision the failing template from the rhev UI on the system you are running against and see if it fails there.

Tried from Rhev UI provisioning failed.

Since the error is coming from rhev provider there is nothing we can do except fail the provision.

The customer that ran into this issue decided not to open an issue with the RHEV-M team.  They created new templates which fixed the problem for them.  

It would be a good idea to see if the RHEV team is aware of this issue and open a new BZ with them if they are not.  Also link any new bug ticket back to this one.

I would suggest closing this ticket as either "Not a bug" as it is not a bug in the ManageIQ code or if you prefer "Can't Fix".
