Description of problem:
Empty image box appears next to fired alert on Monitor->Alerts->All Alerts page. See attachment 

Version-Release number of selected component (if applicable):
5.9.7

How reproducible:
100%

Steps to Reproduce:
1. Fire an alert on an appliance
2. Navigate to Monitor->Alerts->All Alerts page

Actual results:
There is an empty box of an unloaded image next to the provider under which the alert fired.

Expected results:
The image should load properly or not be there at all (this is the case in 5.10.x)

Additional info:
A similar issue was reported for the Monitor->Alerts->Overview page (https://bugzilla.redhat.com/show_bug.cgi?id=1657400), but this issue is on the Monitor->Alerts->All Alerts page.

Please assess the impact of this issue and update the severity accordingly.  Please refer to https://bugzilla.redhat.com/page.cgi?id=fields.html#bug_severity for a reminder on each severity's definition.

If it's something like a tracker bug where it doesn't matter, please set the severity to Low.

Verified that the empty images are not present in 5.10. Since this fix will not be back-ported to 5.9, I am marking this BZ as verified.
