Description of problem: The current labels for the add provider modal form doesn't differentiate between provider types, leaving the user to wonder what exactly username/pwd/hostname is referring to. Let's make the form field labels more descriptive to remove any confusion.

How reproducible: 


Steps to Reproduce:
1. Go to the providers page
2. Click "Add provider" button
3. Notice the labels are the same for hostname/username/pwd regardless of whether you select oVirt or vmware.

Actual results: The labels are the same


Expected results: The brand/provider prefix should be prepended to the field labels.

Verified on MTV build 2.1.0-44 IIB 96817.
