Description of problem: The current labels for the add provider modal form doesn't differentiate between provider types, leaving the user to wonder what exactly username/pwd/hostname is referring to. Let's make the form field labels more descriptive to remove any confusion. Version-Release number of selected component (if applicable): How reproducible: Steps to Reproduce: 1. Go to the providers page 2. Click "Add provider" button 3. Notice the labels are the same for hostname/username/pwd regardless of whether you select oVirt or vmware.
