Description of problem:

On swapin, pages are always assigned to the root anon_vma.  This can lead to pageout scalability issues when a large number of exclusively owned pages get swapped out and back in.

Need to backport my own 3 patches for this, as well as Kamezawa Hiroyuki's "Fix false positive BUG_ON in __page_set_anon_rmap" patch.

This is an optimization, I'm not sure how important this is.

This is my adapted patch that won't need the fix for bug-on.

This request was evaluated by Red Hat Product Management for inclusion
in a Red Hat Enterprise Linux maintenance release. Product Management has 
requested further review of this request by Red Hat Engineering, for potential
inclusion in a Red Hat Enterprise Linux Update release for currently deployed 
products. This request is not yet committed for inclusion in an Update release.
