make exclusively owned pages belong to the local anon_vma on swapin

Description of problem: On swapin, pages are always assigned to the root anon_vma. This can lead to pageout scalability issues when a large number of exclusively owned pages get swapped out and back in. Need to backport my own 3 patches for this, as well as Kamezawa Hiroyuki's "Fix false positive BUG_ON in __page_set_anon_rmap" patch.
