Description of problem:

The only field that is showing up is the "Title" field.  I can't access the fields: 
- Code
- Applies To
- Issue
- Resolution

Tested this issue on devenv_2621, I can not reproduce it, the above fields all shows correctly.

Users who were admins but not moderators could not create or edit KB articles.

Tested this issue on devenv_2644, the codes have been merged to this devenv, and I can create a knowledge base with admin account successfully. The create page shows as my attachment.
