I have had a shell crash, in my case was after a pop up of integrated IBM Lotus Sametime in Lotus Notes 9. I only have three extensions active YAWL, Alternative Activities, and Coverflow Alt-Tab.
Same issue as Luis Verissimo. No gnome extensions installed.
