Description of problem:
There is no separation between create a Template(base template) and create subversion template. 

In new template dialog, when a 'Create as a Sub Template version' check box is enabled the fields that related to create Template (base template - Name,Description,comment,cluster) should be greyed out and contain the base template values.

It is wrong for creation a subversion template the those fields are editable

i dont agree here, you should be able to have all these (description, comment, cluster) for template version.

the only thing in question is the name since there is also version-name so it could be a little redundant
