How reproducible:
100%

Steps to Reproduce:
1.Launch virt-manager.
2.Right click the connection->"Details"->"Add storage pool",choose type as "logical"->"Forward".
3.There is a new input box about "Source Name",click the blank input box,on the right box the description of it is wrong:it is about "Target path" and "Source path".
4.The check box of "Build pool" shows too far,on virt-manager-0.9.0-19.el6.x86_64,the check box is beside "Build pool".

Actual results:
As description.

Expected results:
Correct description of "Source Name" shows and check box of "Build pool" shows normal.

The bug can also be reproduced for gluster fs pool.

I can reproduce with package:
virt-manager-0.9.0-20.el6.x86_64

verified on package:
virt-manager-0.9.0-22.el6.x86_64

steps:
1. Launch virt-manager.
2. Right click the connection->"Details"->"Add storage pool", choose type as    "logical"->"Forward".
3. There is a new input box about "Source Name", click the blank input box,on the right box the description is as follwing:
Source name: Name fo the Volume Group
4. The check box of "Build pool" is besides as normal.
5. Checked with gluster fs pool as Comment 2, display correct as step 3.

so move to verified.
