Description of problem:
In the add host dialog, the tab index jumping between text fields is unexpected - name jumps straight to address (skipping comment, which is placed between them).

Also - comment jumps to name (one above it).

Steps to Reproduce:
1. Open add host dialog
2. Select Name text field
3. Press tab

Actual results:
Skips comment field

Expected results:
Should jump to next field according to GUI layout
