Description of problem:
The dialog to upload a glossary entry has lost its resizing, so appears very small. It's almost unusable.

How reproducible:
Easy always

Steps to Reproduce:
1. Sign in as admin, click Glossary in the menu bar
2. Press Upload
3. Select a file to upload

Actual results:
Terribly crushed dialog

Expected results:
Good dialog
