linux pmda: ensure full set of vm counters represented

We should ensure that all the kernel VM counter data from places such as /proc/sys/vm /sys/devices/.../node/*/meminfo are available in PCP mem.* metrics. (Most of them already are.) Further, there should be a mechanical means of catching any new counters that might appear in future kernel versions, and notifying us to update the PMDA.
