The link you provided leads to the Hypervisor Deployment Guide, which was not in the RHEV 3.4 docs suite:

This was not included in the list of guides agreed to during the RHEV 3.4 development cycle. I am not able to get more information about this, because the correspondence recording the chain of custody on this decision is no longer available to us. Jodi Biddle's correspondence and expertise would make it possible for us to answer this question completely.

Was this bug provoked by a particular case that we can address?

Julie, it looks like the first step in this is to port over Section 4.2 (about 9 topics) 1:1 into the Installation Guide under the section on manual installation.
