Various additional edits. Last edits finished. Also: * Reworked subtitle/abstract/intro to move intent from GSG to Packstack guide. * Restructured/moved 'remove' appendix up into a chapter. * Moved 'prepare' chapter sections into 'requirements' chapter, and restructured.
