Description of problem:
Responsive page rendering fails in Service Catalog Item / Request Info page

How reproducible:
Very

Steps to Reproduce:
1. Navigate to Services ==> Catalogs ==> Catalog Items ==> Add a New Catalog Item ==> Choose a Catalog Info Type.
2. Click on Request Info and observe the bottom of the page in Catalog, Environment and Hardware tabs

Actual results:
Note section is not rendered correctly

Expected results:
Page should render properly
