Issue description:
Description of Custom Login Modules includes note about AuthenticationManager. However for correct understanding, fully-qualified name (FQN) for AuthenticationManager should be used at least for first occurance of AuthenticationManager in text. FQN is org.jboss.security.AuthenticationManager.

Suggestions for improvement:
Mention FQN org.jboss.security.AuthenticationManager in text.
