This sounds like a vestige of old HA issues.  It is not a Keystone bug, but rather a case of split brain operations in the controller nodes, perhaps due to the PKI of PKI tokens, or non-shared token databases.

The customer portal issue has been closed.  At this point, there is no way to either reproduce or confirm fixes.
